What documents should I bring?
What is mandatory and what might be useful:
-
- The exam cannot take place if you do not bring ID
- Ideally, you should bring your passport, as it is always accepted by Canadian authorities
- However, your passport may be stolen, out of date or in the process of being renewed. Should the case arise, bring any other ID document (national ID card, residence permit, refugee card, driver’s license, birth certificate…). Dr AUBERT will scan and submit your identity document to the Canadian authorities, which may then contact you if they feel they need additional documentation.
-
Some prospective immigrants might have received a document called “Medical Examination Instructions” or “IMM 1017” or “IMM 1020” in the mail. If you have received nothing it doesn’t matter, but if you do, it’s imperative that you bring it along, as it contains your “EMR number”, which serves as identifying you in the e-medical software.
In order for you to better understand what it is, here is what it looks like:
-
- If you have a medical condition, bring all the documents you have, including your prescriptions, recent complementary exams and any document that may seem relevant
- Dr. AUBERT will read them, and will add them to your file, should they be relevant
- If you don’t have any medical documents, don’t worry, come anyway!
-
- Dr AUBERT is equipped with e-medical software and will take a picture of you. There is no need to spend extra money on a picture!
